As a new
client you are required to fill in a brief and confidential pre-exercise questionnaire
summarising prior or current health conditions or injuries. If you wish to print the form now please click

Form. Please arrive 10 mins early for your first
class if you need to complete a form. Wear comfortable clothing ie stretch shorts, t-shirt, tracksuit. Try not to eat 2 hrs prior to class. It is
important you continue to update your teacher before class of any recent injury,
illness, surgery or pregnancy. As a student it is important that you take responsibility and be aware of your own personal limitations during practice, approach all exercises and postures consciously using the guidelines given by the teacher. Nicole
